PATIENTS THAT QUALIFY
+ Multi-Level Fusion (3 or more vertebra involved, ie; L4-S1)
+ Pseudoarthrosis (At least 9 months out from previous surgery)
+ Revision Spine Fusion
+ Single-Level Fusion with Risk Factors (Non-Medicare Patients Only)
REQUIRED DOCUMENTS TO PROCESS AUTHORIZATION
+ Prescription for Bone Stimulator (for RX Template Click Here)
+ Patient Demographics (contact info, insurance policy info)
+ Most Recent Clinic Note (must be within prior 6 months)

+ Once the order is received, an Optima Surgical team member will contact your patient and explain the stimulator & process.
+ We submit all order documentation to insurance authorization dept.

ORDERING A BONE STIMULATOR FOR NON-SPINE PATIENTS

PATIENTS THAT QUALIFY
+ Nonunion of a long bone fracture. (Nonunion MUST be documented by a MINIMUM of 2 sets of radiographs separated by a minimum of 90 days, and a written interpretation by a provider stating that there has been no clinically significant evidence of fracture healing between the two sets of radiographs)
+ Failed Fusion of a Joint where a minimum of 9 months has elapsed since the last surgery.
+ Congenital pseudoarthrosis
